				<title>Resumen</title>
				<sec>
					<title>Introducción</title>
					<p> La obesidad en la población pediátrica ha ido aumentando de manera progresiva y el manejo médico de la obesidad mórbida en esta población se ha mostrado insuficiente. El objetivo de este artículo fue describir el estado actual de la literatura científica publicada hasta abril 15 de 2021 sobre el impacto de la cirugía bariátrica en la población pediátrica. </p>
				</sec>
				<sec>
					<title>Métodos</title>
					<p> Se realizó una revisión de la literatura que evaluó dos bases de datos, donde se incluyeron revisiones sistemáticas, reportes de casos, series de casos y ensayos clínicos, publicados desde el 1 de enero de 2010 hasta el 15 de abril de 2021, en español, inglés y francés. </p>
				</sec>
				<sec>
					<title>Resultados</title>
					<p> Se incluyeron 56 documentos y 18 estudios clínicos, en los que se encontró que la cirugía bariátrica (bypass gástrico por Y de Roux, gastrectomía en manga, plicatura gástrica y banda gástrica) alcanzó una pérdida de peso significativa en el primer año, con un adecuado sostenimiento de dicha pérdida al quinto año, además de una resolución completa de las comorbilidades metabólicas y cardiovasculares en la mayoría de los pacientes. </p>
				</sec>
				<sec>
					<title>Conclusión</title>
					<p> La cirugía bariátrica se presenta como una alternativa eficaz en el manejo de la obesidad mórbida y sus complicaciones en el paciente pediátrico.</p>

				<title>Abstract</title>
				<sec>
					<title>Introduction</title>
					<p> Obesity in the pediatric population has been increasing progressively and the medical management of morbid obesity in this population has proven insufficient. The objective of this article was to describe the current state of scientific literature published until April 15, 2021 on the impact of bariatric surgery in the pediatric population. </p>
				</sec>
				<sec>
					<title>Methods</title>
					<p> A literature review was performed searching into two databases, including systematic reviews, case reports, case series and clinical trials from January 1, 2010 to April 15, 2021, in Spanish, English, and French languages. </p>
				</sec>
				<sec>
					<title>Results</title>
					<p> 56 documents and 18 clinical trials were included. It was found that bariatric surgery (Roux-en-Y gastric bypass, sleeve gastrectomy, gastric plication, and gastric band) achieved significant weight loss in the first year, and adequate maintenance of weight loss at the fifth year, in addition to a complete resolution of metabolic and cardiovascular comorbidities in most patients. </p>
				</sec>
				<sec>
					<title>Conclusion</title>
					<p> Bariatric surgery is an effective alternative in the management of morbid obesity and its complications in pediatric patients.</p>

			<title>Introducción</title>
			<p>La obesidad es la principal epidemia con la que se enfrenta el mundo entre las patologías no transmisibles. Es tal el impacto, que se ha estimado una prevalencia de obesidad de 22-24 % en los países desarrollados, y un promedio a nivel mundial cercano al 5 % <xref ref-type="bibr" rid="B1"><sup>1</sup></xref>. En 2018, según los datos de la Encuesta Nacional de Examen de Salud y Nutrición (NHANES), en Estados Unidos, la población entre 2 y 19 años presentaba una prevalencia de sobrepeso de 16,1 %, obesidad 19,3 % y obesidad severa 6,1 %, que afectaba principalmente la población masculina, con un aumento de un 2,5 % respecto a la década anterior <xref ref-type="bibr" rid="B2"><sup>2</sup></xref>.</p>
			<p>En América Latina y el Caribe, con base en los reportes de la UNICEF y la OPS, la prevalencia está en ascenso. En 2016, para el grupo de 5 a 9 años, fueron descritas una prevalencia de sobrepeso del 33,7 % y de obesidad del 15,4 %, y para el grupo etario entre los 10 y 19 años, una prevalencia del 29,1 % y 10,3 %, respectivamente, lo que representa 12,2 millones de niños entre 5 y 9 años con sobrepeso y 11,3 millones de adolescentes con obesidad <xref ref-type="bibr" rid="B3"><sup>3</sup></xref>.</p>
			<p>De acuerdo con los CDC, la obesidad en pediatría es definida como un índice de masa corporal (IMC) por encima del percentil (p) 95 para la edad y el sexo, con base en las tablas de crecimiento generadas por dicha entidad. Hay varios factores que han impactado en el desarrollo de esta patología, como las variaciones en la dieta, dadas por un menor consumo de frutas y verduras en cambio de productos de mejor sabor, pero de poca calidad nutricional, como productos fritos, y bebidas altamente azucaradas, que alcanzan un consumo que representa hasta un 15 % de las necesidades calóricas diarias <xref ref-type="bibr" rid="B4"><sup>4</sup></xref>. De igual manera, el incremento en el uso de la tecnología, dado por el uso de videojuegos y de la televisión, además del estudio desde casa, que han conllevado a una vida sedentaria, con un impacto significativo tanto a corto como a largo plazo en la calidad de vida de adolescentes <xref ref-type="bibr" rid="B5"><sup>5</sup></xref>.</p>
			<p>Flegal y colaboradores <xref ref-type="bibr" rid="B6"><sup>6</sup></xref> caracterizaron los extremos de la obesidad en los mayores de 2 años, definiendo la obesidad severa en un paciente pediátrico con al menos un índice de masa corporal en el (p)99 para la edad y el sexo. También derivaron en una subclasificación que incluye obesidad clase I (p95-p120), obesidad clase II (p120-140) y obesidad clase III (p&gt;140). En la población adolescente, de acuerdo con la descripción de Koebnick <xref ref-type="bibr" rid="B5"><sup>5</sup></xref>, obesidad severa corresponde a un IMC ≥ 35 kg/m<sup>2</sup>, que es el comparativo de la obesidad grado II en adultos. En los pacientes menores de 2 años, según Morales y colaboradores <xref ref-type="bibr" rid="B7"><sup>7</sup></xref>, la definición de obesidad se lleva a cabo usando la relación del peso para la talla, considerando sobrepeso el tener un peso entre el 110 % y 119 % con respecto al peso ideal o un peso para la talla &gt; 2 DS, obesidad si el peso se encuentra entre el 120 % y 129 % o un peso para la talla &gt; 3 DS y, obesidad severa al presentar un peso &gt; 130 % con respecto al ideal.</p>
			<p>La poca disponibilidad de fármacos para el manejo de la obesidad severa en pediatría y su bajo impacto, ha promovido la cirugía bariátrica como opción terapéutica en estos pacientes, con la finalidad de disminuir los riesgos asociados, que incluyen el desarrollo temprano de aterosclerosis, la resistencia a la insulina, prediabetes y diabetes mellitus, el síndrome de apnea/hipopnea obstructiva del sueño (SAHOS), hígado graso no alcohólico (NASH) y múltiples problemas a nivel musculoesquelético, como dolor articular, tibia vara, deslizamiento capital femoral y fracturas patológicas <xref ref-type="bibr" rid="B8"><sup>8</sup></xref><sup>,</sup><xref ref-type="bibr" rid="B9"><sup>9</sup></xref>.</p>
			<p>Si bien, hay sociedades que han validado esta opción terapéutica <xref ref-type="bibr" rid="B9"><sup>9</sup></xref>, algunas consideran que su implementación es algo extremo, por lo que el objetivo de esta publicación fue evaluar, con base en la literatura disponible, los efectos postoperatorios de la cirugía bariátrica en el peso, y su impacto a nivel metabólico y cardiovascular.</p>
		</sec>
		<sec sec-type="methods">
			<title>Métodos</title>
			<p>La presente revisión sistemática exploratoria utilizó la metodología descrita por Arksey y O’Malley <xref ref-type="bibr" rid="B10"><sup>10</sup></xref> y Levac <xref ref-type="bibr" rid="B11"><sup>11</sup></xref>, que corresponde a 5 pasos que incluyen: (I) identificación de la pregunta de investigación, (II) investigar los documentos sobresalientes, (III) escoger los estudios y publicaciones, (IV) obtener los datos, y (V) sintetizar e informar los resultados. Esta revisión respondió a las preguntas: ¿Cuáles son los efectos postoperatorios de la cirugía bariátrica en el paciente pediátrico?, ¿Cuál es el impacto metabólico y cardiovascular generado tras la cirugía bariátrica en el paciente pediátrico?</p>
			<p><italic>Criterios de inclusión</italic></p>
			<p>Los criterios de inclusión fueron: (i) publicaciones teóricas (revisiones sistemáticas y metaanálisis) o estudios clínicos; (ii) la publicación investiga o discute el rol de la cirugía bariátrica en el impacto del manejo de la obesidad en pacientes pediátricos; (iii) la publicación presenta resultados de investigación sobre la implementación de la cirugía bariátrica en el manejo de complicaciones metabólicas y/o cardiovasculares; (iv) el idioma de las publicaciones era inglés, francés o español.</p>
			<p>Se consideró evaluar la evidencia de las dos últimas décadas, con última actualización a abril 15 de 2021.</p>
			<p><italic>Estrategia de búsqueda</italic></p>
			<p>La búsqueda incluyó las bases de datos PubMed y SCOPUS. De igual manera, se incluyeron registros de estudios en 18 bases de datos de Plataforma de Registros Internacionales de Ensayos Clínicos de la OMS: Chinese Clinical Trial Registry, U.S. National Library of Medicine, ISRCTN, EU Clinical Trials Register (EU-CTR), Japan Primary Registries Network (JPRN), Australian New Zealand Clinical Trials Registry (ANZCTR), Brazilian Clinical Trials Registry (ReBec), Clinical Research Information Service (CRiS) - Republic of Korea, Clinical Trials Registry - India (CTRI), Sri Lanka Clinical Trials Registry, Cuban Public Registry of Clinical Trials (RPCEC), Lebanese Clinical Trials Registry (LBCTR), German Clinical Trials Register (DRKS), Iranian Registry of Clinical Trials (IRCT), The Netherlands National Trial Register (NTR), Thai Clinical Trials Registry (TCTR), Pan African Clinical Trial Registry (PACTR) y Peruvian Clinical Trial Registry (REPEC). Estos registros de estudios clínicos cumplen criterios específicos de contenido, calidad y validez, accesibilidad, capacidad técnica y administrativa <xref ref-type="bibr" rid="B12"><sup>12</sup></xref>. Se usaron operadores booleanos y palabras clave de acuerdo con cada sistema de datos.</p>
			<p><italic>Selección de estudios y extracción de datos</italic></p>
			<p>Se utilizó la aplicación web de acceso gratuito para la gestión de revisiones sistemáticas, Rayyan <xref ref-type="bibr" rid="B13"><sup>13</sup></xref>, en la que los autores revisaron y escogieron independientemente los títulos y resúmenes de las publicaciones candidatas. Los investigadores resolvieron discrepancias mediante discusión y consenso. Posteriormente, removimos los duplicados y obtuvimos los documentos seleccionados en texto completo a través de las librerías de la Universidad de la Sabana y la Universidad Nacional de Colombia.</p>
			<p>Se crearon dos formatos con base en las variables que responderían la pregunta de investigación, un formato para los registros de ensayos clínicos y otro para los documentos restantes. Se utilizó el método descriptivo-analítico sugerido por Arksey y O’Malley <xref ref-type="bibr" rid="B10"><sup>10</sup></xref><sup>,</sup><xref ref-type="bibr" rid="B11"><sup>11</sup></xref> para recolectar información estándar de cada documento. Primero, se ajustó el formato usando 5 % de los documentos para determinar si nuestro método de extracción de datos era adecuado. A continuación, se extrajo la información de todos los documentos restantes.</p>
			<p>Para la extracción de la información, cuando estaba disponible, se definieron las siguientes variables: autores, tipo de documento (comentario, estudio clínico, revisión sistemática, metaanálisis), objetivo, revista, país de los autores, hallazgos principales. En el caso de los registros de ensayos clínicos, se recolectó la siguiente información: código de identificación, estado de reclutamiento, diseño del ensayo, país, tamaño de muestra, intervención, control, ocurrencia primaria, fecha de inicio o de registro, fecha anticipada de terminación y fuente.</p>
			<p><italic>Síntesis y presentación de los resultados</italic></p>
			<p>Se presentan los resultados de la revisión siguiendo las categorías propuestas por Grudniewicz y colaboradores <xref ref-type="bibr" rid="B14"><sup>14</sup></xref>: (i) un resumen de las características y distribución de las publicaciones incluidas, y (ii) una síntesis narrativa de los resultados. En este artículo, seguimos la extensión PRISMA para reportar revisiones sistemáticas exploratorias (PRISMA-ScR) <xref ref-type="bibr" rid="B15"><sup>15</sup></xref> (<xref ref-type="fig" rid="f1">Figura 1</xref>). </p>

			<title>Resultados</title>
			<p>Se incluyeron un total 74 resultados, siendo 18 de estos estudios de ensayo clínico, y 56 publicaciones teóricas, que corresponden a estudios de cohorte prospectiva o retrospectiva (n=48), revisiones sistemáticas (n=5), series de caso (n=1) y reportes de caso (n=2). Las características de los documentos incluidos se encuentran en la <xref ref-type="table" rid="t1">Tabla 1</xref>.</p>

			<p><italic>Estudios clínicos</italic></p>
			<p>Se obtuvieron 18 estudios de ensayos clínicos, de los cuales 12 se realizaron en Estados Unidos, y los 7 restantes en Bélgica, Suecia, Francia, Alemania, Italia y Arabia Saudí (<xref ref-type="table" rid="t2">Tabla 2</xref>). El total de participantes en estos ensayos correspondió a 11.681 pacientes. Hay tres estudios (NCT00776776 <xref ref-type="bibr" rid="B98"><sup>98</sup></xref>, NCT01700738 <xref ref-type="bibr" rid="B100"><sup>100</sup></xref> y NCT00289705 <xref ref-type="bibr" rid="B99"><sup>99</sup></xref>) cuyo estado actual es desconocido, y el estudio NCT03139877 <xref ref-type="bibr" rid="B94"><sup>94</sup></xref> aparece en la <italic>U.S. National Library of Medicine</italic> como culminado, sin embargo, en el centro de vigilancia de estudios, aparece sin resultados y sin reclutamiento activo <xref ref-type="bibr" rid="B91"><sup>91</sup></xref>. El estudio que mayor población evalúa (NCT04136704) <xref ref-type="bibr" rid="B75"><sup>75</sup></xref> hará un seguimiento de hasta 10 años en la pérdida total de peso con base en su peso prequirúrgico.</p>

			<p>Cinco de los estudios (NCT01045499 <xref ref-type="bibr" rid="B89"><sup>89</sup></xref>, NCT01980758 <xref ref-type="bibr" rid="B83"><sup>83</sup></xref>, NCT02137330 <xref ref-type="bibr" rid="B81"><sup>81</sup></xref>, NCT02952170 <xref ref-type="bibr" rid="B74"><sup>74</sup></xref>, NCT03620773 <xref ref-type="bibr" rid="B79"><sup>79</sup></xref>) corresponden a ensayos clínicos de intervención sin aleatorizar, uno a intervención abierta prospectiva (NCT04128995 <xref ref-type="bibr" rid="B73"><sup>73</sup></xref>), y los demás son estudios clínicos prospectivos observacionales (NCT00776776 <xref ref-type="bibr" rid="B87"><sup>87</sup></xref>, NCT00474318 <xref ref-type="bibr" rid="B86"><sup>86</sup></xref>, DRKS00004195 <xref ref-type="bibr" rid="B85"><sup>85</sup></xref>, NCT02004561 <xref ref-type="bibr" rid="B84"><sup>84</sup></xref>, NCT03587727 <xref ref-type="bibr" rid="B80"><sup>80</sup></xref>, NCT03203161 <xref ref-type="bibr" rid="B77"><sup>77</sup></xref>, NCT01508598 <xref ref-type="bibr" rid="B76"><sup>76</sup></xref>), retrospectivos (NCT04136704 <xref ref-type="bibr" rid="B75"><sup>75</sup></xref>, NCT04766801 <xref ref-type="bibr" rid="B78"><sup>78</sup></xref>) y de casos y controles (NCT00721838 <xref ref-type="bibr" rid="B72"><sup>72</sup></xref>, NCT03139877 <xref ref-type="bibr" rid="B82"><sup>82</sup></xref>).</p>
			<p>Un 44 % de los estudios tuvo como fin evaluar la mejoría y/o resolución de las patologías cardiovasculares y metabólicas, específicamente diabetes mellitus tipo 2, EHNA, hipertensión arterial y nefropatía asociada; los demás estudios evalúan la variación de peso, en términos de kilos perdidos y/o variación del IMC, con reporte de seguimientos a los 3 meses (3/10), 6 meses (5/10), 12 meses (7/10), 24 meses (2/10) y mayor a este periodo (2/10).</p>
			<p>El estudio NCT01045499 <xref ref-type="bibr" rid="B89"><sup>89</sup></xref>, con un total de 127 pacientes, evalúo el uso de la banda gástrica, su pérdida de peso, la incidencia de depresión y mejoría en la calidad de vida; reportando una pérdida del 1 % del IMC al mes del procedimiento, el 4,8 % a los 6 meses y el 9,8 % a los 12 meses de seguimiento, con una mejoría en la calidad de vida con base en el PedsQL (<italic>Pediatrics Quality Life Inventory</italic>) que aumentó un 13 % en 15 meses de seguimiento (de 76 % a 89 %, p&lt;0,05), con una disminución del 37,0 % en la depresión (p&lt;0,05). Sin embargo, tras el procedimiento, hubo una mortalidad por todas las causas del 2,17 % (3 pacientes) y una tasa de eventos adversos del 5 %, los más comunes de carácter leve, como emesis 70 %, dolor abdominal 48 % y pirosis 44 %.</p>
			<p>Por su parte, el estudio NCT01980758 <xref ref-type="bibr" rid="B83"><sup>83</sup></xref>, evalúo la implementación de la plicatura gástrica en 4 pacientes adolescentes (entre 14-19 años), encontrando una pérdida de peso a los 3 meses de 17,7 ± 7,0 kg (13-28,8 kg), a los 24 meses de 37,6 ± 20,7 kg (17-55 kg) y a los 36 meses de 45,7 ± 34,3 (21,5-70,0 kg); adicionalmente reportaron una disminución del diámetro abdominal de 7,3 ± 6,3 cm (1-14 cm) a los 3 meses y de 34,8 ± 19,9 cm (21-49 cm) a los 36 meses. A nivel metabólico, se reportó al mes 36, una disminución del LDL de 122 a 85 mg/dL, una disminución en triglicéridos de 114 a 55 mg/dL, y un aumento leve del HDL, de 46 a 50 mg/dL. No había alteración de la glucosa previa y no hubo una variación estadísticamente significativa de la tensión arterial.</p>
			<p>En el estudio NCT02137330 <xref ref-type="bibr" rid="B81"><sup>81</sup></xref>, fueron evaluados 17 pacientes pediátricos (9,9 a 17,1 años) con obesidad severa u obesidad moderada con comorbilidades cardiovasculares y/o metabólicas, utilizando una terapia menos invasiva, con la colocación de un balón intragástrico, que se asoció con una dieta de 1100 kcal/día y ejercicio, con seguimiento a los 15, 30 y 90 días tras el procedimiento y después del retiro del balón. Se encontró una pérdida de peso de 9,8 ± 18,4 Kg (Intervalo de confianza [IC<sub>95%</sub>]: 86,01 - 105,58) hasta 83,6 ± 27,1 (IC<sub>95%</sub>: 69,15 - 98,04, p&lt;0,05), con una disminución en el IMC de 35,27 ± 5,89 kg/m<sup>2</sup> (IC<sub>95%</sub>: 32,13 - 38,40) a 32,25 ± 7,1 (IC<sub>95%</sub>: 28,46 - 36,03, p&gt;0,05), y una disminución en el diámetro abdominal de 109 ± 12,3 cm a 99 ± 10,5 cm (p&lt;0,05).</p>
			<p>El estudio NCT04136704 <xref ref-type="bibr" rid="B75"><sup>75</sup></xref> fue uno de los que mayor población evaluó respecto a la implementación de la gastrectomía vertical en pacientes obesos, que al año de seguimiento en pacientes de 14 años o menos, tuvo una disminución del 63,8 ± 14,1 % (p=0,019) del peso excesivo, y que alcanzó un 75,2 ± 12,7 % al quinto año de seguimiento (p=0,07). Respecto al IMC, al primer año hubo una disminución de 30,1 ± 4,6 kg/m<sup>2</sup> (p&lt;0,01) y que alcanzó un 29,7 ± 3,5 al quinto año de seguimiento (p&lt;0,99). Un punto importante de la evaluación incluyó la ganancia de altura, determinando que prequirúrgico presentaban una media de altura de 147,8 ± 14,5 cm, con un z-score de altura de 0,8 ± 1,3, y al quinto año tuvieron una ganancia de 14,5 ± 3,2 cm, contra 12,3 ± 6,4 cm del grupo control, y dicha ganancia fue mayormente reflejada en el grupo etario de 5 a 8,9 años (p&lt;0,01). En relación con las patologías asociadas, en la totalidad de los pacientes se resolvió la diabetes mellitus, y en relación con otras patologías evaluadas (SAHOS, HTA, dislipidemia), hubo un mayor período de control, con resolución de estas, sin diferencia estadística si se realiza dicho procedimiento en la adultez (p=0,72-0,99).</p>
			<p><italic>Síntesis de los hallazgos de las publicaciones incluidas en la revisión</italic></p>
			<p>Dentro de la revisión se seleccionaron 56 publicaciones teóricas (43 PUBMED, 13 SCOPUS), donde la gran mayoría provienen de Estados Unidos (n=20), seguida de Francia (n=7), Arabia Saudí (n=4), Qatar (n=3), Suecia (n=3), Reino Unido (n=2), Canadá (n=2), Alemania (n=2), Portugal (n=2), Italia (n=1), Irán (n=1), Israel (n=1), China (n=1), Bélgica (n=1), Kuwait (n=1), Austria (n=1), Brasil (n=1), México (n=1), Singapur (n=1) y Holanda (n=1); y, de estas, se eligieron las más representativas y que integran el mayor número de pacientes (n=6802), cuyos hallazgos se resumen en la <xref ref-type="table" rid="t3">Tabla 3</xref>.</p>

			<title>Discusión</title>
			<p>La obesidad pediátrica se ha considerado como una de las principales epidemias del siglo XXI, puesto que 1 de cada 3 niños en Estados Unidos es afectado por esta patología <xref ref-type="bibr" rid="B92"><sup>92</sup></xref>. Adicionalmente, trae consigo múltiples consecuencias, incluyendo enfermedades respiratorias, cardiovasculares, metabólicas y neoplásicas <xref ref-type="bibr" rid="B93"><sup>93</sup></xref>, por lo que el manejo oportuno ha tomado cada vez más importancia.</p>
			<p>Actualmente, el tratamiento de esta condición es multidisciplinario <xref ref-type="bibr" rid="B92"><sup>92</sup></xref><sup>,</sup><xref ref-type="bibr" rid="B94"><sup>94</sup></xref>, incluyendo terapia de comportamiento y cambios en la dieta. En este aspecto, hay muy pocos estudios publicados en población pediátrica <xref ref-type="bibr" rid="B95"><sup>95</sup></xref>, que han mostrado que la dieta hipoglucémica tiene un impacto significativo en la pérdida de peso, más que una dieta baja en grasa (2 unidades del IMC, p&lt;0,05). Hay más información en población adulta, en quienes se ha propuesto incluso la incorporación de dietas vegetarianas para el control comorbilidades, como la diabetes mellitus <xref ref-type="bibr" rid="B96"><sup>96</sup></xref>, pero, la evidencia sigue siendo escasa para recomendar algún tipo de dieta específico; además, hay dificultad en la formulación de la actividad física, y del acompañamiento médico a esta.</p>
			<p>En Colombia, se desarrolló un estudio en el hospital San Vicente Fundación <xref ref-type="bibr" rid="B97"><sup>97</sup></xref>, que incorporó un plan multidisciplinario, que incluyó ejercicio en relación con terapias de rehabilitación cardíaca, con seguimiento mensual, y demostró una buena adherencia terapéutica, además de educar al paciente y su familia; con una disminución del Z del IMC en 0,43 que, aunque siendo un cambio significativo, puede llegar a ser insuficiente. </p>
			<p>A nivel farmacológico, las herramientas disponibles son muy pocas <xref ref-type="bibr" rid="B8"><sup>8</sup></xref><sup>,</sup><xref ref-type="bibr" rid="B95"><sup>95</sup></xref>, con el Orlistat como único fármaco aprobado por la FDA en pacientes ≥ 12 años, y la Sibutramina <xref ref-type="bibr" rid="B95"><sup>95</sup></xref> en pacientes ≥ 16 años. Otros manejos complementarios que han mostrado favorecer el control y disminución del peso, son la Metformina, el Exenatide y el Topiramato; pero, aún con esto, el manejo conservador de la obesidad mórbida pediátrica se ha mostrado insuficiente.</p>
			<p>Con base en esta revisión de los diferentes ensayos clínicos y revisiones sistemáticas se muestra cómo la cirugía bariátrica, en cualquiera de sus modalidades quirúrgicas (derivación gástrica en Y de Roux, gastrectomía en manga vertical o banda gástrica ajustable) es eficiente para el tratamiento de la obesidad mórbida en adolescentes, que, al ser comparada contra los adultos, los resultados en la población pediátrica son más significativos, en relación con una mayor pérdida de peso al primer año, que se ha mantenido más al ser evaluada al quinto año, y con una mayor tasa de resolución de comorbilidades metabólicas y cardiovasculares.</p>
			<p>Al detallar la implementación de las diferentes técnicas quirúrgicas, se encuentra una mayor reducción del IMC y mayor porcentaje de remisión de comorbilidades posterior a la realización de la derivación o baipás gástrico <xref ref-type="bibr" rid="B16"><sup>16</sup></xref><sup>,</sup><xref ref-type="bibr" rid="B18"><sup>18</sup></xref><sup>,</sup><xref ref-type="bibr" rid="B23"><sup>23</sup></xref>, seguido por la gastrectomía en manga <xref ref-type="bibr" rid="B19"><sup>19</sup></xref><sup>,</sup><xref ref-type="bibr" rid="B21"><sup>21</sup></xref><sup>,</sup><xref ref-type="bibr" rid="B22"><sup>22</sup></xref> y, en último lugar, la banda gástrica <xref ref-type="bibr" rid="B21"><sup>21</sup></xref><sup>,</sup><xref ref-type="bibr" rid="B22"><sup>22</sup></xref><sup>,</sup><xref ref-type="bibr" rid="B23"><sup>23</sup></xref>, sin embargo, se ha documentado menor morbilidad quirúrgica y menor riesgo de déficit de micronutrientes tras la implementación de la gastrectomía en manga frente a otras técnicas, por lo que su uso es mucho más amplio.</p>
			<p>Pese a ser documentada la utilidad de la cirugía bariátrica en el manejo de la obesidad pediátrica, su implementación ha sido controversial, especialmente por los servicios clínicos (pediatría y medicina familiar), quienes apelan al manejo médico y limitan la remisión al servicio de cirugía pediátrica en hasta un 48 % <xref ref-type="bibr" rid="B98"><sup>98</sup></xref>. De hecho, se han hecho estudios que han evaluado las actitudes de pacientes, familiares (padres) y profesionales en salud en relación con esta situación <xref ref-type="bibr" rid="B99"><sup>99</sup></xref>, que demuestran el impacto y la falta de información con la que se maneja esta alternativa terapéutica. Además, se ha demostrado que los médicos de atención primaria y los pediatras carecen de cierta información en relación con el manejo de la obesidad en pediatría, además de la poca confianza que tienen frente a su capacidad para generar cambio en la población pediátrica, y especialmente adolescente <xref ref-type="bibr" rid="B100"><sup>100</sup></xref>, factores que impactan directamente en la atención de estos pacientes.</p>
		</sec>
		<sec>
			<title>Limitaciones</title>
			<p>Dentro de las limitaciones encontradas, se tiene que únicamente se incluyeron dos bases de datos o índices citacionales, correspondientes a la base de datos más nutrida del mundo, y la otra, con mayor número de resúmenes y citaciones. Además, tiempo de publicación se limitó a las dos últimas décadas y solamente contempló tres idiomas, lo que deja por fuera publicaciones que pueden dar mayor peso a la presente publicación. En relación con los documentos y estudios clínicos, dado el tipo de población y la patología evaluada, las cohortes no son grandes, y los seguimientos, en general fueron limitados con base en la edad adulta establecida por la normativa de cada país. Cabe destacar que, por el tipo de publicación, la calidad de la evidencia no fue tomada en cuenta, dado que no es el objetivo de las revisiones sistemáticas exploratorias, puesto que la pregunta de investigación es poco específica <xref ref-type="bibr" rid="B10"><sup>10</sup></xref>.</p>
		</sec>
		<sec sec-type="conclusions">
			<title>Conclusión</title>
			<p>La cirugía bariátrica se ha convertido en una alternativa terapéutica para el manejo de la obesidad mórbida en la población pediátrica, ya que es segura, eficaz en lograr y mantener una pérdida sostenida de peso y, ofrece un amplio porcentaje de resolución de comorbilidades, sin embargo, hay que tener en cuenta que este manejo deberá contar con un equipo multidisciplinario, que involucre a los padres, familiares, pacientes y profesionales en cuestión, para crear una red de apoyo sólida. En relación con los ensayos clínicos, estos deberán realizarse con cohortes más grandes, y el tiempo de seguimiento debería ser más largos, incluyendo la adultez temprana, para dar mayor soporte científico a su aplicación. Es destacable también, que esta temática debería ser incluida en el currículum tanto de pregrado médico, como en la residencia de las especialidades afines, dado el continuo aumento en volumen de pacientes con esta patología y la necesidad de dar un manejo efectivo a la misma.</p>
